Dr. Tate has moved out of state in June 2023.
My last day of work will be this coming May 31, 2023. I will be moving to South Carolina and plan to practice there. I am looking forward to the next chapter in my life but will miss all my patients, staff and working with Costco. It has been a pleasure taking care of and working with all of you.
All patient files for this office will be left in the custody of Doctor Kristan Huff at this location, therefore continuity of care will not be disrupted.
